Case Management Series Part 1: Foundations of Case Management & Documentation January 2025

This comprehensive training will cover the process of case management by introducing necessary skills and supportive tools to utilize in providing best practices related to case management, including documentation tips. This training day is designed to give a basic foundation of case management to attendees with an emphasis on people who have never done case management. The history, theories, and practice of case management will all be covered here.

Learning Objectives:

  1. Participants will be able to define case management and understand the basic functions of a case manager.
  2. Participants will be able to identify the four phases of case management.
  3. Participants will be able to identify at least two types of case management and populations they are most effective in supporting.
