How do we know when someone is done with case management? For both new and experienced case managers, the elements of monitoring progress will be explored, as well as evaluation. Participants will be able to expand their practice and knowledge of the final phase of case management. Learning Objectives:
Participants will be able to identify three different tools/ways to monitor progress during this case management phase.
Participants will define at least two strategies to adjust the plan when needed.
Participants will understand the transition process and be able to identify at least three signs of readiness for the Transition phase.
