Case Management Series Part 4: Service Planning April 2025

The writing of a service plan will be covered in this training and both new and experienced case managers will be able to benefit from the information presented. Participants will practice writing needs and outcomes statements and developing planning strategies for case management.

Learning Objectives:

  • Participants will be able to explain the process for helping a client define their vision.
  • Participants will be able to define what an outcome statement is and how to utilize the SMART test.
  • Participants will be able to identify the zones of usefulness and explain enabling versus empowering.
